      We report an 81-year-old man with a history of hypertension and stage 4 chronic kidney disease. A posteroanterior chest radiograph showed mediastinal widening with a visible left heart border (Figure 1A, arrowheads; Figure 1C, arrowheads). This widening corresponds to the posterior aneurysm as demonstrated on both transverse (Figure 1C, arrow) and sagittal (Figure 1D, arrow) sections of the computed tomographic (CT) scan. From the posteroanterior view, the sizable mass and the left heart border can be demarcated (Figure 1A, arrowheads), thus we can conclude that the mass lies posterior when coupled with a lateral film (Figure 1B).
